You will need to upload images of the following documents, so be sure to have digital images handy (can be taken with your phone):

• A copy of your child's birth certificate

• A copy of your child's immunization records (or letter of religious or medical exemption)

• A photo of your child

STEP 1: ONLINE ENROLLMENT FORMS

• Enrollment Packet

  Please complete as soon as possible. The enrollment packet is lengthy and will take some time to complete. It is mobile-friendly, but due to the length, it is recommended that you use a desktop computer or laptop to fill it out.

• 5th - 8th Grade Student Narrative

  Only for new students in 5th - 8th grade; must be completed by the student.

STEP 2: PAPER FORMS

• PRINT OUT: Affidavit of Parent/Legal Guardian
  Must be notarized. Student Services & Data Manager Kelly Stephenson is a notary and can notarize this if needed.

• PRINT OUT: North Carolina Health Assessment Transmittal Form

  This form is only for new students entering a North Carolina Public School for the first time (applies to most kindergartners. It must be filled out by a health provider.

STEP 3: ANNUAL FORMS

• Annual Forms Packet

  You will be automatically redirected to this packet when you submit your Enrollment Packet, so by the time you reach step 3, you've likely already completed this form.
